Torque wrench calibration nearly cost me a frame last week

Last Tuesday I torqued a stem bolt to spec with my Park Tool TW-5, snapped the bolt clean off, and the bar rotated into the top tube before I caught it. Turns out the wrench was 30% off after 3 years of daily drops in the van. Sent it in for calibration, the tech said he sees this constantly, especially with beam type heads. Has anyone else had a trusted torque wrench drift that bad, or am I just unlucky with my abuse?
